Admissions

Service Users will only be admitted following referral by a Care Manager from one of the Health and Social Care Trusts and completion of a pre admission assessment by a Senior Nurse. The purpose of the preadmission assessment is to ensure that we can safely and effectively meet the needs of the person and to ensure the home is registered to accommodate their assessed needs. The pre-admission assessment ensures all equipment is available in advance and should bespoke training be required for staff this is also delivered in advance.
